Central CCAC

The Central Community Care Access Centre serves 1.9 million people who reside in the communities of North York, South Simcoe County and York Region. Our region is one of the most populous and diverse in the province. Close to half of the people who live here are new Canadians, many are visible minorities. They speak Cantonese, Russian, Punjabi, Italian, English, or one of a dozen other languages, and they look to Central CCAC when they need health care services delivered at home and in their communities.

Our goal is to get people the care they need, at home in the community. In 2015-2016, we delivered care and services to over 86,000 people – supporting over 38,000 people on any given day. We provided just over 3.6 million hours of personal support to help people with complex health issues manage their personal care. We also safely transitioned over 39,000 patients to the comfort and familiarity of home once they no longer needed to stay in the hospital.

It takes a diverse set of skills and people to provide the best care possible at home and in the community. Central CCAC is always looking for caring and skilled health care professionals, such as care coordinators and nurse practitioners to support the unique needs of our patients. Our teams are also made up of business related positions, such as decision support specialists, risk management experts, quality improvement analysts and project managers.
