Care Coordinators

Community Care Access Centres (CCACs) play a key role in Ontario’s health care system. Working in partnership with patients, families, providers and community organizations, CCACs ensure people have access to the health care they need — at home and in the community.

Our Care Coordinators come from various work backgrounds, including nursing, physiotherapy, occupational therapy, social work and other health care-related fields. You’ll draw on your expertise to work in the community, at local hospitals or at one of our CCACs as a liaison between patients and various health care providers to help them achieve optimal health, independence and dignity. See below for specific specialties by location.

You’ll have the freedom and flexibility to enjoy a balanced lifestyle, while still fulfilling your passion of providing meaningful care to patients and their families. In return, we offer a supportive and respectful team culture, the stability of full-time hours, as well as excellent benefits and vacation allowance.
