Health Care Managers
Community Care Access Centres (CCACs) play a key role in Ontario’s health care system. Working in partnership with patients, families, providers and community organizations, CCACs ensure people have access to the health care they need — at home and in the community.
Health Care Managers lead teams in delivering, and facilitating access to, coordinated quality care within a hospital or the community. As a hospital team lead, you will support hospital-based Care Coordinators who help transition patients from the hospital to other health care facilities, so they can enjoy life again in the right place of care. A Community Team lead is responsible for Care Coordinators who conduct face-to-face assessments with patients in the community, and connect them to the right community-based supports and care.
Your experience working in either a hospital, health centre or community environment is paired with exceptional people management skills, and the ability to build and develop strong relationships with both colleagues and external partners.
