Nurse Practitioners

Community Care Access Centres (CCACs) play a key role in Ontario’s health care system. Working in partnership with patients, families, providers and community organizations, CCACs ensure people have access to the health care they need — at home and in the community. CCACs employ Health Care Managers, Care Coordinators, Nurse Practitioners, and more.

Our Nurse Practitioners serve as clinicians, educators, collaborators and advocates. In this role, you will work as an integral member of the Primary Care Team, assisting in building team capacity to provide care and clinical management, engage in shared care with primary and specialist providers to address more complex care needs, and facilitate access and transition to and from tertiary services.
